The Ultimate Document Checklist for Your Passport Appointment

Don't get sent home! From "Fresh Applications" to "Name Changes," here is exactly what you need to carry to the PSK.

The number one reason people are turned away at the Passport Seva Kendra (PSK) isn't because of a background check—it's because of missing or mismatched documents.

The rules change depending on your age, marital status, and employment. At Smile Foreign Education, we have compiled this exhaustive guide to ensure you walk in prepared for any scenario.

📜 The 3 Golden Rules

  1. Originals are Mandatory: You must show the original document for verification. No photocopies, no digital scans (unless DigiLocker is explicitly accepted).
  2. Self-Attested Copies: Carry one set of photocopies for every document. You must sign them yourself.
  3. Data Match: Your Name, Father's Name, and DOB must match exactly across all documents (e.g., Aadhar vs. Marksheet). Even a spelling error can cause rejection.

Scenario 1: Fresh Passport (Adult)

This is the most common category. You generally need to prove three things:

3. Non-ECR Proof

10th Class Marksheet/Passing Certificate (Most critical doc!)

• 3 Years of Income Tax Returns (ITR)

Scenario 2: Minors (Children < 18)

Minors require documents from their parents.

  • Proof of Date of Birth: Birth Certificate (Mandatory).
  • Proof of Address: Parent's Passport (Original) or Parent's Aadhar.
  • Annexure D: A declaration signed by both parents accepting responsibility.
  • Parent Presence: Ideally, both parents should be present. If one is abroad, Annexure C or a sworn affidavit is required.

Scenario 3: Re-issue / Renewal

If your passport is expiring, expired, or pages are exhausted.

  • ✔ Old Passport: You MUST carry the original old passport. (They will cancel it and return it to you).
  • ✔ Self-Attested Photocopies: First two pages and last two pages of the old passport.
  • ✔ Address Proof: Only needed if your address has changed. If the address is the same, no proof is needed (select "No Change" in the form).

Scenario 4: Name Changes

💍 Due to Marriage

To add Spouse Name: Marriage Certificate is highly recommended.
However, if you don't have it, a joint affidavit (Annexure K) or updating your Aadhar to include the spouse's name often works.

📝 General Name Change

Major Change: 1. Gazette Notification (Publication in Govt Gazette).
2. Newspaper Advertisements (1 Local + 1 National).
3. Two witnesses.

Scenario 5: Tatkaal Applications

Tatkaal is faster (1-3 days dispatch) but stricter. You generally cannot use Tatkaal if you have an adverse police report history or complex changes.

The "3 Document" Rule:

For Tatkaal, you typically need to provide 3 separate documents from the list of 13 acceptable documents (e.g., Aadhar + Pan + Voter ID).
Note: Minors and Senior Citizens often have relaxed rules for Tatkaal.

Scenario 6: Students & Renters

Living away from home?

  • Rent Agreement: Must be a Registered Rent Agreement (Notarized is usually rejected).
  • Photo Passbook: A bank passbook of a running account in a PSU/Private bank with your current address and photo attested by the bank manager is the safest bet.
  • Student ID: A Bonafide Certificate from a recognized university can sometimes act as supporting evidence for address.
